Restiamo per un attimo sul tema dell\u2019ESP e segnaliamo, come promesso, l\u2019uscita a stampa della narrative review di Chin ed El Boghdadly su Canadian <strong>(Chin, K.J., El-Boghdadly, K. Mechanisms of action of the erector spinae plane (ESP) block: a narrative review. Can J Anesth\/J Can Anesth (2021)<\/strong>. <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33403545\/\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/pubmed.ncbi.nlm.nih.gov\/33403545\/\u00a0<\/a> che per\u00f2 non ci solleva da tutti I dubbi finora emersi, anzi li rinforza, dimostrando nuovamente come si stia utilizzando a largo spettro una tecnica della quale non si conosce ancora il meccanismo d\u2019azione. Senza indugi Jeff Gadsden scrive una reply sullo stesso numero, <strong>(Gadsden, J. The erector spinae plane block: the case of the elusive mechanism of action. Can J Anesth\/J Can Anesth (2021<\/strong>).<br \/>\n<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33403541\/\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/pubmed.ncbi.nlm.nih.gov\/33403541\/<\/a> ) nella quale sottolinea proprio tale problematica, esortando I ricercatori a concentrarsi, per il prossimo futuro, sui meccanismi dell\u2019ESP e non soltanto sulla sua efficacia, facendo la gara a chi ne fa l\u2019applicazione pi\u00f9 stravagante.<\/p>\n<p>Chiuso, per ora, il capitolo ESP, che riprenderemo <strong>nel nostro appuntamento del 24 Aprile<\/strong> (<a href=\"https:\/\/www.esraitalia.it\/index.php\/corsi-e-congressi\/\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/www.esraitalia.it\/index.php\/corsi-e-congressi\/<\/a>), non possiamo non segnalare lo special issue di gennaio di Anaesthesia, completamente dedicato all\u2019anestesia loco regionale. Schreiber<\/strong> <strong>The role of regional anaesthesia and multimodal analgesia in the prevention of chronic postoperative pain: a narrative review<\/strong> First published: 10 January 2021 <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33426669\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/pubmed.ncbi.nlm.nih.gov\/33426669<\/a>.<br \/>\n<img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-1254\" src=\"https:\/\/www.esraitalia.it\/wp-content\/uploads\/2021\/02\/Immagine1-300x149.jpg\" alt=\"\" width=\"437\" height=\"217\" srcset=\"https:\/\/www.esraitalia.it\/wp-content\/uploads\/2021\/02\/Immagine1-300x149.jpg 300w, https:\/\/www.esraitalia.it\/wp-content\/uploads\/2021\/02\/Immagine1-1024x508.jpg 1024w, https:\/\/www.esraitalia.it\/wp-content\/uploads\/2021\/02\/Immagine1-768x381.jpg 768w, https:\/\/www.esraitalia.it\/wp-content\/uploads\/2021\/02\/Immagine1.jpg 1077w\" sizes=\"auto, (max-width: 437px) 100vw, 437px\" \/><br \/>\nSullo stesso argomento RAPM pubblica un\u2019altra review limitata alla chirurgia oncologica della mammella, in cui il ruolo dell\u2019anestesia locoregionale si rileva importante, in particolare nel prevenire l\u2019insorgenza di dolore cronico di tipo neuropatico (<strong>Hakim Harkouk, Dominique Fletcher, Valeria Martinez<\/strong> <strong>Paravertebral block for the prevention of chronic postsurgical pain after breast cancer surgery<\/strong> January 07, 2021 <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33414157\/\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/pubmed.ncbi.nlm.nih.gov\/33414157\/<\/a>.<\/p>\n<p>Per concludere, sempre da RAPM, ci sembra interessante segnalare questa original research, (Christopher Wahal, Stuart A Grant, Jeffrey Gadsden, Milly T Rambhia, W Michael Bullock <strong>Femoral artery block (FAB) attenuates thigh tourniquet-induced hypertension: a prospective randomized, double-blind, placebo-controlled trial<\/strong> January 11, 2021<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33431616\/\" target=\"_blank\" rel=\"noopener noreferrer\"> https:\/\/pubmed.ncbi.nlm.nih.gov\/33431616\/<\/a> (e cliccandoci) gli autori dimostrano come la risposta ipertensiva da tourniquet possa essere inibita da un\u2019infiltrazione di anestetico locale attorno all\u2019arteria femorale. Tale conclusione, apparentemente di scarsa rilevanza, in realt\u00e0 evidenzia come i plessi nervosi perivascolari abbiano un ruolo fondamentale nella trasmissione \u201cnocicettiva\u201d (nel caso specifico del dolore di tipo ischemico, ma anche del dolore da manipolazione vascolare), di come il sistema nervoso autonomo sia ampiamente coinvolto in tali situazioni e di come i blocchi periferici (talvolta anche quelli centrali) non possano da soli essere sufficienti per un\u2019analgesia completa. Ne consigliamo la lettura perch\u00e9 riteniamo l\u2019argomento interessante e stimolante per future ricerche. Di seguito l\u2019abstract.<\/p>\n<p>&#8220;<strong>Introduction<\/strong> Prolonged tourniquet inflation during surgery frequently leads to tourniquet hypertension (TH), which is thought to arise from compression of A-\u03b4 fibers leading to sympathetically mediated C fiber activation. In the lower extremity, C fibers and other sympathetic nerve fibers are carried along the femoral artery. We hypothesized that blockade of these fibers at the femoral artery would decrease the incidence of TH.<\/p>\n<p><strong>Methods<\/strong> Thirty American Society of Anesthesia 1\u20133 patients aged 18\u201375 undergoing total ankle arthroplasty were randomized to receive 15\u2009mL of injectate (mepivacaine 1.5% or saline placebo) at the anteromedial aspect of the common femoral artery at the level of the inguinal crease under ultrasound guidance. Both groups received preoperative popliteal sciatic and saphenous nerve blocks for analgesia and a standardized general anesthetic. Esmolol was administered if systolic blood pressure rose &gt;30% above baseline. Incidence of TH was the primary outcome.<\/p>\n<p><strong>Results <\/strong>TH was present in 93.3% of sham patients versus 33.3% of block patients. Mean systolic pressure at 120 min and 150\u2009min of tourniquet time was significantly higher in the sham group compared with the block group. Esmolol requirement (95.3+107.6\u2009v 8.0+14.2, p=&lt;0.001) was also significantly higher in the sham group. No differences were noted in pain scores or opioid consumption, and no patient experienced sensory or motor block of the femoral nerve.<\/p>\n<p><strong>Discussion<\/strong> Under these experimental conditions, injection of local anesthetic around the femoral artery reduced the incidence of TH and intraoperative esmolol requirement.\u201d[\/vc_column_text][\/vc_column][\/vc_row]<\/p>\n<\/div>","protected":false},"excerpt":{"rendered":"<p>[vc_row][vc_column][vc_column_text]Anche in questo numero della newsletter, \u00e8 stato impossibile limitare la scelta ad un solo articolo.
